  Request for an Appointment

On this page, your potential client fills in the main personal information required in the Appointment Request Form: name, email address, phone number. Any optional information (address, fax number, etc.) might be also required. Boxes for clients’ messages to you are included in this Form. Here, the client can mention days and times when he could come for a visit. Your work schedule can be shown on this page too. After a client clicks “Send” button, this information is transmitted to your office by email. Then, your assistant can propose the client some specific day and time in the Reply email or call the client by phone to finally agree about the date and time.

 

Price for this function is $100 - $160 (depending on your Internet host provider).   

 

Questionnaire Forms

This option allows the client to fill in and print out the questionnaire forms at home before coming to your for appointment.  In this case you avoid waiting for the client until he completes the form in your office. This option will also allow the client to send his completed questionnaire forms to your office by email.

Price is from $150 for each form.

 

Questionnaire Forms to Be Sent to Your Database

This option provides sending the Questionnaire Forms filled by your clients to your database for further analysis. In this case, their completed Questionnaire Forms will be saved in your database. Then, you can generate and printout these Forms yourself whenever you want. Possibility of printing these Forms by your clients at their homes remains in this case as an option.

Price is from $200 for each form.

 

Moving Ad on Your Welcome Page

This option allows you to publish current ads, such as information about specific temporary changes in your schedule, about your coming vacation, etc. Due to its moving feature, such ad draws attention of your clients. We give you a user-friendly Instruction on how to insert or substitute the text in the Moving Ad.  This function might be supported by database that will store Ad context.

Price of this function is from $100. Price for each additional Moving Ad on the same website is $25.
